Summary

add --heartbeat-file and --no-heartbeat monitor flags
wire ANGLERFISH_MONITOR_HEARTBEAT_FILE and ANGLERFISH_MONITOR_NO_HEARTBEAT env configuration
document the new heartbeat controls and cover them in CLI tests
