What changed

• Adds acceptance_failed for failed delivery and release-validation test steps.
• Marks the affected path as failed and emits a targeted repair suggestion.
• Updates taxonomy documentation and adds a regression derived from the real GitHub sandbox fault trajectory.

Root cause

run_acceptance_tests returned failed, but the analyzer only recognized generic error text and therefore reported the path as PASS.

Validation

• Full local suite: 76 passed.
• The recorded sandbox fault now reports acceptance_failed.
• llm-eval-engine independently holds the same failed business-state Episode.
