Add medication clinician context reports

What changed

• Add a local, range-scoped clinician report with clinical-day sleep charts, drift, explicit-record adherence, medication start/dose markers, and non-causal before/after context.
• Add redaction-first, typed-confirmed standalone HTML export with no scripts, external assets, sync, or network path.
• Add the strict clinical chart request contract, ADR-0027, roadmap updates, and optional civil medication start editing.

Safety and privacy

• Absence of a medication log is never classified as a missed dose.
• Diagnosis, location details, and clinician-entered guidance are always omitted.
• Private labels and both note classes remain separate opt-ins.
• Custom ranges constrain chart, drift, association, and context inputs.
