feat(man): harden planning and verification

What changed

• make /man clarify decision-changing requirements until they are ready, with recommendations and no fixed question count
• add plan-only, governed execution, and lightweight solo handoff paths
• add structured requirement coverage and acceptance criteria ledgers
• add automated/manual/hybrid verification gates, manual confirmation, and remediation re-verification
• add explicit review skipping with recorded reasons and strengthen completion gates
• update CLI help, agent/skill templates, lifecycle docs, website copy, and release version to 0.3.8

Why

The previous workflow could plan before requirements were sufficiently aligned and could complete despite unverified manual behavior or stale verification evidence. These changes make planning readiness and delivery evidence explicit without forcing every user into the full governed execution path.
