Add accepted-state delivery guidance

Summary

• add shared accepted-state delivery guidance to V3 bootstraps and legacy/minimal/Head Coach templates
• specialize Man completion, commit, and PR copy plus Manteam handoff, commit, and PR copy
• preserve failures, blockers, excluded scope, migrations, audit evidence, and formal handoff states and reasons
• extend compiled-distribution checks across generated bootstrap, Man, and Manteam adapters

Compatibility

• no CLI, schema, mode, ledger, or completion-gate changes
• keep the V3 adapter version at 3
• existing managed adapter content becomes stale and continues through the normal dry-run and confirmed upgrade flow

• 104 focused contract tests passed
