Add S001 seed-user intake form

What changed

• add one structured GitHub Issue Form for the S001 concierge experiment
• reduce participant intake to a public task link, repeated-agent-path description, agent/frequency/recency, and consent
• make verification command and failing commit optional so AEG can infer them
• require explicit public-license, privacy, and participation acknowledgements

Why

AEG has enough verified product evidence to recruit seed users, but no external adoption evidence. The current S001 CTA has ambiguous intake and asks participants for details AEG can infer. A single GitHub Issue Form creates an observable, auditable funnel without building a landing page, backend, or demo video.

This does not launch recruitment or claim AEG improves repairs. Publishing recruitment copy still requires founder approval.

Validation

• parsed as YAML with Python/PyYAML
• 11 form blocks
• 10 unique field IDs
• no labels, assignees, external writes, credentials, or private-data collection
