Fix Claude project path resolution

Summary

• Resolve Claude project folder names through ~/.claude.json projects before falling back to legacy dash-to-slash decoding
• Share project path encoding helpers between project summaries and session listing/detail parsing
• Cover hyphenated project paths and legacy fallback behavior with unit tests
