Spec r1 + r1.1 split (issue comment 6464291699; approved 6464308234). Chief signed off §5 with conditions (a)-(e); this PR is (a). PR (b) (private care bucket, read route, care content check) follows and is blocked by this one.
drops the init-schema INSERT, UPDATE and public-read policies and creates editor images owner folder insert (only editor/{own tenant}/{auth.uid()}/{non-empty rest}), editor images owner folder update (owner, and the new name stays in the owner's folder) and editor images owner select (owner only), all TO authenticated. The INSERT swap (C1) and the read-policy swap (C3) are in the same migration (Chief condition a). The owner delete policy is unchanged.
sets file_size_limit = 5242880 and the four image types; public is unchanged, so public object URLs keep working.
records the prior limits in private.storage_bucket_limits_20261028100000 (first record wins; REVOKE ALL from PUBLIC, anon, authenticated, service_role; no grants anywhere in the migration).
The rollback restores the three original policies exactly as in init_schema and the recorded limits, then drops the table, all in one transaction. It raises and changes nothing if the recorded row is missing.
POST /api/upload/editor-image (admin and missionary apps), shared handler packages/api/src/upload/editor-image.ts:
uses the caller's bearer client (never the service role); the server picks the key editor/{ctx.tenantId}/{ctx.userId}/{uuid}.{ext}, with the extension taken from the detected type; uploads with upsert: false; returns 201 { url, path }.
purpose must be exactly feed, care-thread or care-note. feed needs missionary/staff/admin/super_admin; care purposes need staff/admin/super_admin.
packages/api/src/editor-images/key.ts: the purpose set and path builder. PR (b) adds the key grammar.
Editor:LegacyRichTextEditor gets an imagePurpose prop. With it, the editor posts FormData { file, purpose } with the session bearer to the route and inserts the returned url; it no longer calls storage.from(...). Without it (and without onImageClick) there is no upload button (OQ19, fails closed). The four mounts pass string literals: care thread care-thread, private care note care-note, org updates feed, worker feed feed.
Docs:docs/security/storage-editor-images.md (policies, limits, recorded-limits table, key layout, pre-deploy listing, deploy, rollback); runtime-map rows for both upload mounts.
CI: appends tests/unit/acceptance/issue-2407/editor-images.realstack.acceptance.test.ts as the last path of "Verify tenant isolation (real stack)", and re-pins PINNED_STEP_BODIES to match.
Mount identity
The handler module exports two handlers built by one private factory: adminPOST (feed + care purposes) and missionaryPOST (feed only). Each app route stays a one-line re-export, as with /api/upload/image:
Why: the route files stay single re-exports with no logic, and what each mount accepts is fixed in the shared package next to the role rules. There is no plain POST export that accepts care purposes, so a future mount has to pick one of the two handlers explicitly. The missionary refusal runs before any storage call (criterion 23, Chief condition d).
The tenant segment check in editor images owner folder insert (and the folder part of the update policy) is copied verbatim from #2406's 20261027100000_profiles_bucket_owner_folders.sql (introduced in e2e6a753f394, at builder/2406 head 9563beea769f):
(string_to_array(name, '/'))[2] IN (
SELECT COALESCE(
p.tenant_id::text,
CASE WHEN p.role='super_admin' THEN '00000000-0000-0000-0000-000000000001' END
)
FROMpublic.profiles p
WHEREp.user_id= (SELECTauth.uid())
)
The bucket id and the purpose segment (= 'editor') are the only parts of the policy that differ from #2406's.
Interim care behaviour (until PR b)
The care thread and private note mounts already send care-thread / care-note. On the admin mount the route accepts them from staff, admin or super_admin and stores the file in document-uploads under editor/{tenantId}/{userId}/{uuid}.{ext} with a public URL, as the editor does today. There is no care content check yet. The missionary mount refuses both care purposes for every caller. PR (b) moves care uploads to the private care bucket and read-route paths; files uploaded in the meantime stay where they are, and the listing before (b) is applied counts them.
Pre-deploy listing (factory, read-only, only after the owner's go-ahead): objects in this bucket outside the new layout, of another type or over 5,242,880 bytes; the bucket's current limits and policy list, flagging any policy on this bucket that is not in the migrations (that goes back to security sign-off before apply); counts and row ids of care rows embedding this bucket's URLs. The result goes in the factory handoff note, not GitHub. Nothing is moved or deleted.
Order: apply the migration and deploy the apps together. Migration first: the old editor's direct uploads are refused until the apps deploy (posting text still works). Apps first: the new route works under the old policies.
Limits: the rollback restores the recorded prior limits, never NULL; if they differ from the factory listing, restore the listed values.
Manual checks after deploy: worker feed and org updates: insert an image (201), publish, open its URL signed out (200); over-limit file and non-image type rejected (413/415); list() on the bucket signed out returns nothing.
Rollback: restores the earlier upload and read policies, so ship it only together with reverting the app commit, and only for as long as needed.
Both PRs append one path to the same CI step. Whichever merges second re-pins PINNED_STEP_BODIES (tests/unit/helpers/workflow-yaml.ts), issue-2250 STEP_RUN, and the issue-2155 / issue-2201 workflow fingerprints. If #2406 merges first, its real-stack other-bucket policy snapshot also needs the r1.1 re-pin for this bucket's new policies. The compat-bootstrap hunk is identical in both PRs.
The reviewed changes appear safe to merge, with the previous scope-test failure fixed.
Summary
The PR moves editor image uploads through shared authenticated routes and tightens writes and object listing in document-uploads.
Editor images now go through a role-aware server route.
The editor image bucket limits uploads and scopes object access.

…set limits (#2407)
Migration 20261028100000 replaces the document-uploads INSERT, UPDATE and
public read policies with owner-folder INSERT/UPDATE and owner-only SELECT
(the tenant expression is #2406's text), sets 5 MB / four image type limits
and records the prior limits for the rollback. The compat bootstrap gains
the two limit columns, with the same text as #2406.
…#2407)
Shared handler on the admin and missionary mounts: bearer client only,
server-chosen editor/{tenantId}/{userId}/{uuid}.{ext} keys, upsert false,
#2308's type, signature and size checks (now exported from upload/image).
feed needs missionary or staff roles; care purposes need staff and are
refused on the missionary mount. Until PR (b), care images use the editor
image bucket with public URLs.
LegacyRichTextEditor posts { file, purpose } with the session bearer to
/api/upload/editor-image instead of writing to storage from the browser.
The new imagePurpose prop turns uploads on; without it there is no upload
button. The care thread, private note, org updates and worker feed mounts
pass care-thread, care-note, feed and feed.
…ests in CI
Adds docs/security/storage-editor-images.md and the two upload-route
runtime-map rows, and appends the #2407 real-stack file to Verify tenant
isolation (real stack) with the matching PINNED_STEP_BODIES re-pin.

📝 Summary
Adds a shared POST /api/upload/editor-image route for the admin and missionary apps. It checks authentication, upload purpose, user role, and image validity before storing files under server-generated paths.
Enables uploads in feed, care-thread, and care-note editors. Editors without an upload purpose or custom image handler do not show an upload control.
Updates document-uploads policies and limits. The migration sets a 5 MiB limit and allows JPEG, PNG, WebP, and GIF. Care images remain in this bucket.
Adds deployment and rollback guidance, bootstrap compatibility, and unit and acceptance tests. The PR description reports 93 acceptance tests passed and 23 skipped; these results are not independently verified.
Updates the tenant-isolation policy pin and documentation, and fixes real-stack test placeholder expansion.

The migration records existing bucket limits, sets a 5 MB limit and four allowed image types, and applies ownership and folder checks. The rollback restores recorded limits and prior policy behavior. The bootstrap schema and security documentation describe the storage contract. The issue-2256 reader-policy list adds the two editor-image policies.

Explanation
The pull-request diff contains no skill or agent mirror changes. It also contains no generated skill or agent paths requiring a canonical source or sync-tooling update. The generated-mirror condition is not triggered.
Full details: Tenant Safety
Explanation
No tenant-safety failure is introduced. The upload handlers require the bearer-authenticated context, authorize roles, derive the key only from ctx.tenantId and ctx.userId, and use the bearer-scoped Supabase client. The migration restricts INSERT and UPDATE to the caller's tenant/user folder and limits SELECT and DELETE to the object owner. The previous reader-policy and real-stack placeholder defects are addressed: issue-2256/harness.ts and identity-person-anchor.md include both storage policies, and the #2407 harness resolves {demo} before compound placeholders with UUID-compatible segments. Prior failure addressed: Commit 4594624 adds both storage policies to the #2256 reader pin and fixes #2407 placeholder expansion; commit 5a20e33 updates the identity-person-anchor documentation. Current source confirms these changes.

Use private storage for care-note images.
The private-note editor stores public image URLs in member_care_private_notes.content. Anyone who obtains a URL can fetch the image without note access.
The documentation scopes public care images to the period before PR (b), but this remains a privacy exposure. The same issue existed in the merge base because the legacy private-note editor uploaded directly to the public document-uploads bucket.
Note: this behaviour predates this PR (already present at merge base 629cb71dc206); the change keeps it reachable.

Add a forged-owner INSERT check and enforce owner in the INSERT policy.
The inserts() helper always derives owner from the acting user, so no allow or deny case checks a different or NULL owner. The INSERT policy has no owner predicate, so a direct insert with another user's owner would pass. That user's owner-only SELECT policy would then show the row.
Suggested fix
AND (string_to_array(name, '/'))[3] = (SELECT auth.uid())::text
AND (string_to_array(name, '/'))[4] <> ''
+ AND owner = (SELECT auth.uid())
);
Add this line to the INSERT WITH CHECK only (the block that starts at CREATE POLICY "editor images owner folder insert"). Then add a deny case that inserts with a forged owner. That case fails until the policy change is applied.

Add a same-user, other-tenant rename case to updates().
The UPDATE policy checks the tenant segment, but the current rename cases fail on other conditions first. A policy that removes only the tenant check would still pass the existing tests. Add the proposed case to cover this regression.

When #2406 is present but uses equivalent syntax that tenantExpr does not recognize, criterion 22 skips before comparing N1. Fail extraction in this case, but keep the skip when #2406 is absent. This is a test-coverage gap, not a current production-policy failure.

MAJOR (gate-blocking, test harness): the demo-tenant rows in the Editor inline image storage: owner-scoped paths, type and size limits, and access that matches the content #2407 realstack suite never build a valid key, so 5 required realstack cases fail. tests/unit/acceptance/issue-2407/editor-images.realstack.acceptance.test.ts:29-30 expands {editor:T:U} with /\{(\w+):(\w+):(\w+)\}/before{demo} is substituted, so {editor:{demo}:S} (line 40) and {editor:{demo}:S2} (line 52) don't match. After line 30 the object name keeps literal braces, {editor:00000000-…-0001:S}/<uuid>.webp, and RLS correctly refuses it. CI shows expected [ 'allow S into demo: 42501' ] to deeply equal [] for criterion 3 (both plain and c9 hosted-grants runs), the N1 reference self-check, and the M2/M3 mutant probes. Effect: the super_admin to demo-tenant allow path (criterion 3) is unproven, and the S2 into demo denial passes for the wrong reason, so the M2 probe can't catch a "demo tenant for any super_admin" mutant. The policy itself looks right on reading. This is a harness defect, not product behavior, but the spec requires the issue-2407 realstack suite to pass with none skipped, and migrate is red. Fix: substitute {demo} first, and let the tenant group accept a UUID (for example [\w-]+). The CodeRabbit inline thread on line 29 makes the same call, and I agree with it.
Dropped from the automated pass
The rollback-doc point (docs/security/storage-editor-images.md:107, "If the recorded values differ from the pre-deploy listing, restore the listed values") is not a defect. Spec §11 ("If they differ from the factory listing, the rollback restores the hosted limits the factory recorded") and Chief's condition (c) say the same thing. No change needed.
Systems / failure pass (no further product or threat-model defects)
C1 and C3 ship together in this PR. N1 and N2 confine writes to editor/{own tenant or demo for tenantless super_admin}/{auth.uid()}/<non-empty>, and N3 narrows SELECT to owner = auth.uid(). DELETE, other buckets and the public flag are unchanged.
Route (packages/api/src/upload/editor-image.ts): bearer required (401), bearer-scoped client (never service role), requireAuth, purpose allowlist (400), care purposes refused on the missionary mount at the route (403, Chief (d)), role check, capped body read, MIME/size/signature validation, server-chosen random key, upsert: false, generic storage errors.
Bucket limits are recorded in a private table (API roles revoked) before the update, and the rollback raises rather than writing NULL. Everything runs in one transaction.
test-unit: failure, but neither failure is from this PR: issue-2067/migration-lockdown criterion 4 hit an ENOENT on a transient .factory-hosted-test-* temp file on the runner, and scripts/e2e-dev-server.test.ts failed a timing (max_seconds deadline) case. Rerun these.
integration-gate, ci-gate, e2e-smoke-gate: failure, downstream of the above. e2e/smoke jobs were skipped.
lint, typecheck, format, build, integrity, acceptance-tests, compilation-plan: success.
Re-review on the next head is a delta review of the pin/doc and harness changes plus a green migrate.
…tenant_id reader set (#2407)
Review finding 1: the owner-folder INSERT and UPDATE policies on
document-uploads read profiles.tenant_id, so they join the sanctioned
reader set in section 1.3, with their fail-closed rule and the
super_admin demo fallback tracked in #2267. The bucket contract points
at the entry. The policies are unchanged.
Hold note: this PR is waiting on fixes for Reviewer's CHANGES NEEDED at 8c955ce (comment 6479959257). Acceptance owns the READER_POLICIES pin and the realstack harness fix, and Builder owns the doc 1.3 update. Required checks (migrate, ci-gate, e2e-smoke-gate) stay red until then.
Delta re-review of 8c955ce..f2cafe9 (3 commits, 6 files: docs and tests only, no supabase/ changes).
Prior findings (CHANGES NEEDED at 8c955ce, comment 6479959257)
M1 (fixed).READER_POLICIES in tests/unit/acceptance/issue-2256/harness.ts:74-81 now pins storage.objects editor images owner folder insert and ... update alongside the profiles policy, and docs/security/identity-person-anchor.md §1.3 (lines 100-110) lists both policies with their fail-closed behavior and the super_admin fallback tied to [Security] S1 cleanup: profiles.tenant_id authorization readers (follow-up from #2256) #2267. docs/security/storage-editor-images.md:61-62 cross-references the reader set.
M2 (fixed). The realstack case rows and table() moved to tests/unit/acceptance/issue-2407/harness.ts:53-108; {demo} and {id:K} now resolve before the compound {purpose:tenant:user} token, so {editor:{demo}:S} expands to editor/<DEMO>/<S>/... instead of surviving as a braced literal. New no-database tests in static.acceptance.test.ts:429-482 assert no name keeps a brace, pin the five allow names and the M2 probe names, and self-check that the pre-fix order is rejected.

CI at this head
All check runs complete and green, including migrate (realstack: #2256 test 6 and the 2407 realstack suite), acceptance-tests, test-unit, typecheck, test-e2e-smoke, build.
Merge note
#2425 and #2427 edit the same §1.3 bullet of docs/security/identity-person-anchor.md and READER_POLICIES in tests/unit/acceptance/issue-2256/harness.ts. Whichever merges second must keep all five pin entries and all three doc bullets when resolving the conflict.
The criterion 22 COALESCE block is byte-identical to #2425's.
Protected-area sign-off record (storage): Chief signed off for Blake at f2cafe9d0999a258aea3765f17703a770501cbaf only, with conditions. Before merge: Acceptance PASS at this exact head and all required CI green. Whichever of #2425 and #2427 merges second has to carry every #2256READER_POLICIES row and both identity-person-anchor.md §1.3 entries, re-pinned by Acceptance. That re-pin plus green CI needs no new sign-off, but any other change in that delta goes back to Chief. The review of record is Reviewer's delta review (6489606627); the delta 8c955ce..f2cafe9 is docs and tests only. Applying this to the hosted environment is a separate decision, outside this merge.

#2407 PR (a): editor image bucket hardening and editor upload route
Spec r1 + r1.1 split (issue comment 6464291699; approved 6464308234). Chief signed off §5 with conditions (a)-(e); this PR is (a). PR (b) (private care bucket, read route, care content check) follows and is blocked by this one.

PR (b) adds the key grammar.LegacyRichTextEditorgets animagePurposeprop. With it, the editor postsFormData { file, purpose }with the session bearer to the route and inserts the returnedurl; it no longer callsstorage.from(...). Without it (and withoutonImageClick) there is no upload button (OQ19, fails closed). The four mounts pass string literals: care threadcare-thread, private care notecare-note, org updatesfeed, worker feedfeed.docs/security/storage-editor-images.md(policies, limits, recorded-limits table, key layout, pre-deploy listing, deploy, rollback); runtime-map rows for both upload mounts.tests/unit/acceptance/issue-2407/editor-images.realstack.acceptance.test.tsas the last path of "Verify tenant isolation (real stack)", and re-pinsPINNED_STEP_BODIESto match.Mount identity
The handler module exports two handlers built by one private factory:
adminPOST(feed + care purposes) andmissionaryPOST(feed only). Each app route stays a one-line re-export, as with/api/upload/image:Why: the route files stay single re-exports with no logic, and what each mount accepts is fixed in the shared package next to the role rules. There is no plain
POSTexport that accepts care purposes, so a future mount has to pick one of the two handlers explicitly. The missionary refusal runs before any storage call (criterion 23, Chief condition d).#2406 tenant expression (Chief condition b, criterion 22)
The tenant segment check in
editor images owner folder insert(and the folder part of the update policy) is copied verbatim from #2406's20261027100000_profiles_bucket_owner_folders.sql(introduced ine2e6a753f394, atbuilder/2406head9563beea769f):The bucket id and the purpose segment (
= 'editor') are the only parts of the policy that differ from #2406's.Interim care behaviour (until PR b)
The care thread and private note mounts already send
care-thread/care-note. On the admin mount the route accepts them from staff, admin or super_admin and stores the file indocument-uploadsundereditor/{tenantId}/{userId}/{uuid}.{ext}with a public URL, as the editor does today. There is no care content check yet. The missionary mount refuses both care purposes for every caller. PR (b) moves care uploads to the private care bucket and read-route paths; files uploaded in the meantime stay where they are, and the listing before (b) is applied counts them.Deploy notes
init_schemaand creates theprivateschema if missing. It does not need Profile photo storage: scope uploads to the owner's folder, add bucket limits, and restrict profile image URLs to our storage #2406's migration, so it is safe on an environment that lags develop.list()on the bucket signed out returns nothing.Merge order with #2406
Both PRs append one path to the same CI step. Whichever merges second re-pins
PINNED_STEP_BODIES(tests/unit/helpers/workflow-yaml.ts), issue-2250STEP_RUN, and the issue-2155 / issue-2201 workflow fingerprints. If #2406 merges first, its real-stack other-bucket policy snapshot also needs the r1.1 re-pin for this bucket's new policies. The compat-bootstrap hunk is identical in both PRs.Tests
