相较 v1.0.3 的更新

• 修复 VS Code 窗口重载后，旧终端仍持有过期 Router 地址而导致 Hook 超时或通知丢失的问题
• Helper 现在优先使用当前工作区刷新后的 Router 记录，避免通知被发送到其他 VS Code 窗口
• 修复同一个 Codex turn 内多次提问或授权请求时，后续通知被错误去重的问题
• Codex 重复事件现在只在 2 秒内去重，既避免瞬时重复通知，也不会遗漏后续提问
• 完善点击通知跳转说明，并完成本机与 Linux Remote SSH 的端到端实机验证
• 修正 GitHub Actions 发布流程：Tag 只创建 GitHub Release，不再尝试使用无权限的 Marketplace/Open VSX 发布凭据
• 更新项目主页、Issues 地址和 Codex、Remote SSH、Windows Notifications 等检索关键词

• Presenter 和 Router 都安装到 VS Code 的 Local

Remote SSH 使用：

• Presenter 安装到本机 Windows 的 Local
• Router 安装到 SSH: <服务器名>

安装后，在每个已经打开的 VS Code 窗口中执行：

Developer: Reload Window

首次安装再运行：

Remote Notifier: Auto-configure notifications in current workspace for...

并选择 Codex。升级时 Hook 命令路径没有变化，通常无需重新配置或重新信任。
