Add SkillEvaluator metadata.author and fix dead Markdown links

Summary

Fixes NVIDIA SkillEvaluator Tier 1 schema and code-integrity failures observed on this skill tree.

• Adds metadata.author to SKILL.md in the SkillEvaluator Name <email@host> shape. Keeps the existing top-level author: Applied Alchemy Labs / Hermes.
• Retargets relative Markdown hrefs that SkillEvaluator 0.2.0 treats as missing files:
  • Query-string links such as index.md?family=… and ?term=… now point at the existing index.md files (docs/start/see-it-work.md, docs/index.md, docs/map/index.md, and the archive run pages that used the same pattern).
  • QUICKSTART.md → examples/quickstart/ now points at examples/quickstart/README.md.

Verified, not changed

These reported targets already exist at the linked paths, so the hrefs were left as-is:

• QUICKSTART.md → ./README.md, docs/start/*, docs/hermes-skill.md
• SPEC.md → docs/api-v1.md
• ARCHITECTURE.md → ./SPEC.md, ./docs/api-v1.md, ./docs/hermes-skill.md
• docs/phase5.md archive links (archive/index.md, archive/runs/backfill-phase5-rizz-2026/index.md)
• docs/commands.md and docs/operator-loop.md relative links (commands.md, start/quickstart.md, modules/vectordb.md)

No skills/*/SKILL.md copies exist in this repo.

Testing performed

• PYTHONPATH=src python3 -m pytest -q: 196 passed, 5 skipped
• SkillEvaluator 0.2.0 validate --checks schema --no-dedup on a hyperlex/ skill tree: PASS (author_format accepted; no author_missing)
• SkillEvaluator HygieneValidator._check_dead_links: all relative links valid in 98 markdown files

Docs/metadata-only: adds metadata.author to SKILL.md frontmatter and fixes several dead Markdown links (points at examples/quickstart/README.md instead of the directory, strips non-functional ?family=/?term= query-string links in favor of the actual index.md target). No runtime code touched. CI failures (CodeQL, validate, build) also fail identically on main at HEAD, so they're pre-existing/unrelated to this diff, not a regression introduced here.

Closing: CI red since Aug; SkillEvaluator metadata/link fixes are stale vs current org main + personal product tip (mutation/installer). Re-open on a fresh branch from main if still needed after personal→org sync.
