feat(memory): the learning store, its ledger and host calls, and memory filed and read by scope on both runtimes - #620
Continues #451, which shipped memory_traces.scope_id at Brain schema version 3, set by every writer, and the importers' deduplication inside one scope id. This pull request carries the rest of the learning increment.
Learning gets a durable record. With the learning flag on, a learning store keeps each learner's trait row, the run-time freezes and suspensions, a sessions index and the tombstones of forgotten users; an append-only ledger, one hash chain per learner whose heads advance by compare-and-swap, records every memory write the memory tools and the rolling-summary sink make and every operator call, so an edit made outside the layer shows at the next load. Operators freeze, suspend, set, revert, export, import and forget through host calls on AgentOS.getLearningLayer() and agent.learning. Memory a learner owns is filed and read under scope ids that keep tenants, users and sessions apart (the GMI's memory, the memory tools, the standalone retriever and the rolling-summary sink), and every write of a user's memory honours forgetUser's tombstone, writes in flight included. gmi() gets durable memory (memory.store). Every host gets the stability fix behind persistAccessState.
For every host, flag on or off:
A trace's stability, access count, reinforcement interval and next reinforcement time are written at every access and read back at load. A store with a Brain keeps them in the row's metadata.decay, the decay state the Memory facade, its consolidation and the memory tools read and write; a store with no Brain keeps them in its vector metadata (persistAccessState, default true; false keeps the previous behaviour for one release).
A deleted trace takes its knowledge-graph copy with it. Each knowledge graph keeps one copy per trace, keyed by the trace (node id trace:<traceId> in the in-memory and SQL graphs, an indexed traceId on Neo4j), so recording a trace again replaces its copy; a copy an older release recorded gets its key at a graph's first delete by trace id. A delete for good (deleteTraces, deleteScope) removes the copies before the rows, and a graph that cannot delete them fails the call with every row kept, so a later call finds the same traces. With no Brain, deleteScope finds a scope's traces in the vector index, those another store filed before a restart included.
MemoryStore.hydrateScope loads one scope's most recent traces by the scope_id column, so a returning user's older memories are recallable however many traces other scopes hold, and MemoryStore.refileScope moves one scope id's traces to another: every Brain row, soft-deleted rows included, the vectors of the traces the store holds and their knowledge-graph copies, and with no Brain the documents a durable index holds. The Memory facade gains deleteScope, deleteTraces, restoreTrace, readTrace, countScope and brainId, and remember takes extra metadata and an id its caller names (a text the scope holds already answers under the id of the trace that holds it); the package root exports TraceSnapshot and readTraceSnapshot. A trace can carry turnUserId, the second user who filed it on another user's session: deleteScope takes it as a filter, and the facade's deduplication keeps that user's words a trace of their own. The facade's deletes clear the rows that name a trace (its feedback, the chunk it was filed from) and its memory-graph node and edges, whichever facade wrote them; the deletes and the refile refuse an empty scope id.
An import or a RAG migration reads a source's scope-id form from its record (brain_meta.scope_id_form, a JSON export's meta, a note's front matter, each written only where a learning layer opened the Brain) and refuses a source the target cannot take, naming the record (scope_id_form); a source whose scope ids read the same in both forms is taken as a plain one. A Qdrant source or target takes part through its sidecar's brain_meta, a source that names no brain (a Pinecone index) is refused into a target that holds the encoded form, and a read of a form or a clear of a record that fails for another reason than a missing table fails the migration.
conversationManagerConfig.tablePrefix keeps the runtime's conversation tables as <prefix>conversations and <prefix>conversation_messages. The conversation manager refuses a database whose table of either name it did not make (a memory Brain's conversations, or SqlStorageAdapter's), with a CONFIGURATION_ERROR that names tablePrefix, before it changes anything. AgentOS.initialize() refuses a revisioned or sealed provenance policy that protects the conversation tables only by their default names while a prefix renames them, naming the tables to add to storagePolicy.protectedTables.
A Postgres Brain reads its integer columns as numbers, so a trace's counts and times survive a restart.
The memory tools that write, and memory_reflect, refuse with frozen under AGENTOS_LEARNING_FREEZE=1, with a learning layer or without one, before the tool orchestrator's approval gate.
The package root exports the learning stores (InMemoryLearningStore, SqlLearningStore), toolLearningContextFor and the types of the host calls' inputs and reports; the rest of the learning module is on @framers/agentos/cognition/learning.
New optional members on exported interfaces (IKnowledgeGraph.deleteMemories, ICognitiveMemoryManager.deleteScope and deleteTraces, MemoryTrace.turnUserId, RememberOptions.metadata, the learning interfaces), which no implementer has to add.
gmi()'s memory.store needs the learning option and learning.agentKey, and fails at construction naming them without either.
learning: null, and a field of the learning option set to null, read as unset on gmi(), agent({ runtime: 'gmi' }), createLearningLayer() and the full runtime, as a config read from JSON or YAML writes them: learning: null builds no layer, and the option's type takes null.
With the learning flag on:
A learning store keeps each learner's trait row, the run-time freezes and suspensions, the sessions index and the tombstones, and an append-only, hash-chained ledger records every memory write of the memory tools and the rolling-summary sink and every operator call; learning.ledgerKey keys the hashes.
A learner's trait row starts at the persona's authored traits, written with its ledger row at the learner's first session, and every later session checks it against the learner's chain and the digest of its newest ledger row: a record that does not verify renders the authored traits, logs a warning and is counted in the manifest's ledger loop as chain broken. A persona whose traits changed moves the row's baseline (each learnable trait stays within 0.2 of it), and a persona that lowers honesty-humility does not lower it for a learner who had it. The manifest's ledger loop names where the store is and whether the hashes are keyed; a store that does not open renders the authored traits and refuses the writes it would record.
Host calls: freeze, unfreeze, suspend, unsuspend, operatorSet, revert, exportLearnedState, importLearnedState, forgetUser (on AgentOS.getLearningLayer(), and on agent.learning with the agent's key and namespace filled in). Each runs in every mode and under a freeze, and refuses a forgotten user's learner. forgetUser takes the user's write chain, commits the tombstone and deletes the user's traces in every memory manager and memory-tools facade the layer holds (a second user's by turnUserId), then the user's learning rows; its report and the manifest's forgetUnreached name the stores it does not reach, and a session another process holds stops rendering the user's learned traits at its next turn. A recorded memory write a crash left pending is decided at the learner's next session once it is 15 minutes old, and the agent's chains with no learner apart from the session's open; its verdict is written before it is taken back.
A learner whose namespace is set files and recalls its memory under namespaced scope ids. A host that passed organization ids before this release reads its earlier traces for those users after re-filing them with MemoryStore.refileScope, and runs it before it relies on forgetUser for them (until then forgetUser with a namespace leaves their plain-form traces and logs a WARNING).
Every plain id in a user or persona scope that holds a /, a % or a : is filed percent-encoded, and a Brain's earlier traces under such ids are re-filed when a layer first opens it.
A turn with no learner files under its session's own form, which no user id takes; a turn's own conversation and session thread ids are filed and read in that form, so a conversation's thread traces from before the layer are not read.
The GMI id and the memory owner id leave the user recall list; the bridge no longer adds the owner id to the organization list, where a turn's own organization ids stay; an owner id that begins with / or holds // leaves the thread list.
The GMI files each turn's words through a guard that reads the user's tombstone before the write and again once it has committed, takes back a filing whose user was forgotten meanwhile, and feeds the memory observer a filing only once the guard keeps it.
The memory tools, the standalone retriever and the rolling-summary sink file and read under the turn's scope ids; the tools reach no other scope's trace by id, search only the turn's own scopes and the unscoped traces, and refuse an organization write; their writes and the sink's are recorded, on a turn with no learner under its explicit user, or under the session's learner for a write into a learner's own session, and otherwise with no user; a write whose user is forgotten while it runs, in this process or another, is taken back for good; a write of a trace another writer changed or moved to another scope id since the call read it answers conflict; a thread note memory_add files is reached by id from its filer's turns alone, so an anonymous turn on a learner's session and the learner reach none of each other's; only a freeze stops them, and in mode off or under a suspension they write with no row.
memory_reflect is withdrawn; hosts consolidate with Memory.consolidate().
memory.store on gmi() gives durable memory, with the learning store on the same adapter (a PostgreSQL adapter opened with typeParsers: { int8: 'number' }; one that reads BIGINT as text is refused): a session id's memory survives a restart and agent.close(), and session.close() ends it for good, across restarts and processes.
Every process that serves one Brain turns the flag on together, on this release or a later one. In this release a host's Memory.consolidate() merges and derives across scope ids, and forgetUser does not reach the memory archive (its report names it), so products turn the flag on with the release that keeps consolidation inside each scope id and reaches the archive, or a later one.
CI on this pull request (ci.yml): Build, the exports check, Lint, the documentation word check, Type check and Test with coverage, on every push of the branch. MemoryStore.access-state.test.ts runs a real SQLite Brain through a store, an access and a restart, with the Memory facade's feedback on the same row between the access and the restart, and with the switch off; and two stores over one vector store with no Brain. The integration cases run the paths a host takes: gmi() end to end over the real provider manager, GMI, memory manager, Brain and learning layer with only the provider class stubbed (gmi-runtime.learning-store.test.ts); AgentOS.create() end to end with the memory tools, the standalone retriever and the rolling-summary sink on one SQLite adapter and one Brain shared by two tenants (agentos-learning-memory.e2e.test.ts, agentos-learning-tenants.e2e.test.ts), and through the built package on SQLite and Postgres (learning-store-and-scopes.e2e.test.ts); a sealed provenance policy beside the prefixed conversation tables (conversation-tables-provenance.e2e.test.ts); the ledger's CI test, whose path list is read from the source (ledger.integration.test.ts); and the flag-off cases on both runtimes, which pin that a host that leaves the flag off files and reads as before and builds no learning table.
Migration notes
None needed. No change breaks a caller. Every change that reaches a host with the learning flag off is listed in the Summary's first list; everything else needs the flag. A host that turns the flag on reads the second list.

Explanation
Direct issue [#627] requires scope-aware memory filing and retrieval, a learning store with a ledger, and host calls. The reviewed head implements these requirements on both runtimes. LearningStore, InMemoryLearningStore, SqlLearningStore, and the hash-chained Ledger persist learner state and memory-write records. LearningLayer and AgentLearning expose freeze, suspension, trait, import/export, revert, and forgetUser operations. GMI memory, memory tools, standalone retrieval, and rolling-summary writes use learner or session scope IDs. Tests cover store contracts, ledger behavior, host calls, memory tools, both runtimes, tenant isolation, and documented release limits.

…option and keeps its parameters
A SQLite export copies each table in a transaction of its own, as before; an import from a SQLite file passes its one transaction to the copy.
…ss deleted for good
refileScope moved a trace this store held under the old id whose Brain row was gone, deleted for good by another process before the move or while it ran: it wrote the trace's words into the new collection, which a shared index keeps, and recorded its knowledge-graph copy again. With a Brain, a trace with no row is now left as the store holds it and is not counted.
…ords of a trace deleted for good once the re-file committed
The fan-out after a re-file moved each trace a store held under the old id without reading its row. A delete for good another process made the moment the re-file committed (on Postgres such a delete waits on the rows the re-file holds locked) had the trace's words written into the new collection and its graph copy recorded again. The follow now reads the row and leaves a trace whose row is gone as the store holds it.
…ore and after its rows and copies again when a re-file commits between
A brain on one connection, such as an in-memory SQLite database, is read
statement by statement, so a re-file another flow committed between the copy
of brain_meta and the copy of the traces gave a file with no record and
encoded ids, which an import into an encoded Brain encoded a second time.
…nd Postgres
The SQL store runs the learning store contract on SQLite and, where
AGENTOS_TEST_POSTGRES_URL names a database, on Postgres, each Postgres
case in a schema of its own. It leaves a host's adapter open when it
closes, reads a learner's outcomes, lessons and skills by creation time
and id, refuses an adapter without transactions, makes no table when a
product runs its own migrations, makes its tables at its next call when
its first attempt failed, and adds a column a later release names to the
tables an earlier release made, keeping their rows.
The contract gains two cases every store keeps: text is ordered by
UTF-16 code unit whatever order the database sorts text in, and a
transaction's handle refuses every call once its transaction has ended
(moved from the in-process store's own cases).
…ts ten tables
SqlLearningStore keeps learned state in ten tables over a host's storage
adapter, Postgres for a deployment several processes share and SQLite for
one process: the ledger and each chain's head, trait rows, outcomes,
lessons and skills, freezes, sessions, the probe cache and the tombstones
of forgotten users. Every statement is portable between the two: TEXT and
INTEGER columns, ON CONFLICT upserts, and positional parameters only.
Each chain head moves by compare-and-swap on its seq, a trait row by its
version, and every state change commits in one adapter transaction. The
store refuses an adapter without transactions, leaves the host's adapter
open when it closes, and makes its tables before its first statement,
trying again at the next call when that fails; ensureSchema: false leaves
the DDL to a product's own migrations.
A store an earlier release made gains the columns a later release names
when it opens: LEARNING_COLUMN_ADDITIONS, added through the storage
library's ensureColumnExists, so their rows are kept.
…fuses a transaction's handle once it has ended
A database orders TEXT by its collation, and Postgres by the database's
locale, under which agent|a and agent|b sort before agent|C. Freezes, a
user's session rows, a learner's or a candidate's ledger rows and a
learner's documents therefore came back from a Postgres store in another
order than the one the store promises and the in-process store keeps.
The store now sorts them once read, by UTF-16 code unit; a chain's rows
stay ordered by the database, by their integer seq.
A call made on a transaction's handle after the transaction ended ran
outside any transaction: on Postgres on the pool, on SQLite as a
statement of its own, so a handle kept past its function wrote with
nothing to take the write back. The handle now throws, as the in-process
store's does.
• Persist learner traits, operator controls, tombstones, and tamper-evident memory-write records
across sessions.
• File and retrieve memory by tenant, user, and session; make forgetting remove reachable traces.
• Preserve trace access state, protect conversation tables, and cover both runtimes with integration
tests.

High-Level Assessment
The following are alternative approaches to this PR:
1. Put the ledger inside each memory Brain
➕ Could commit a memory write and its ledger entry in one database transaction.
➖ Couples learning state to one memory backend and complicates writes spanning multiple facades or stores.
Recommendation: Keep the separate learning-store contract and pending/verdict protocol: they support both runtimes and memory backends while making incomplete writes recoverable. Do not enable the learning flag for privacy-sensitive deployments until consolidation stays within scopes and forgetting reaches the archive; the PR explicitly identifies those remaining gaps.

1. Forgotten memories remain in the graph✓ Resolved
Description
MemoryStore.softDelete discards failures from knowledgeGraph.deleteMemories, and deleteTraces
then permanently removes the trace row; the facade's deleteTraces likewise ignores graph-deletion
failures after removing its row. If graph deletion fails during forgetUser, the graph copy retains
the trace's full text and remains available through graph recall, while the deleted row no longer
provides a path to retry its cleanup.
+ try {+ await this.config.knowledgeGraph.deleteMemories?.({ traceIds: [traceId] });+ } catch {+ // Non-critical, as recording the copy is (store()).
Evidence
The added catch suppresses graph failures, both permanent-deletion paths continue to remove trace
rows, and graph recall reads its own full-text nodes without consulting those rows.
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution
## Issue description
A failed graph deletion is ignored during permanent trace deletion, leaving a retrievable copy of forgotten content after its trace row is gone.

- src/cognition/memory/retrieval/store/MemoryStore.ts[3139-3155]
- src/cognition/memory/io/facade/Memory.ts[937-957]
## Recommended Fix
For both deletion paths, do not report permanent deletion as complete while a full-text graph copy may remain. Propagate the graph failure before removing the last retryable trace record, or durably record failed graph deletions and retry them independently; test a failing graph backend followed by a retry.

2. Qdrant migrations skip scope checks✓ Resolved
Description
MigrationEngine._scopeIdFormHandshake returns without a decision whenever either adapter lacks
scopeIdForms, but the Qdrant adapters lack that method while still migrating Brain tables from a
SQLite sidecar. A Qdrant migration into a SQL target can therefore copy brain_meta and scoped
graph rows without checking whether their scope-id form is compatible with the target.
The changed guard skips the handshake for adapters without the optional method, while the Qdrant
source enumerates and copies sidecar tables containing scope-form records and graph rows.
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution
## Issue description
The new scope-form handshake silently skips Qdrant migrations even when their sidecars contain the Brain tables it is meant to protect.
## Fix Focus Areas

- src/cognition/rag/migration/adapters/QdrantSourceAdapter.ts[18-22]
- src/cognition/rag/migration/adapters/QdrantTargetAdapter.ts[16-22]
## Recommended Fix
Implement scope-form reads for Qdrant sidecars and run the same handshake used for SQL adapters. Where a backend has Brain tables but cannot provide their form, refuse the migration rather than silently skipping validation.

3. Durable GMI memory on a host Postgres adapter garbles counts✓ Resolved
Description
openAgentMemoryBrain opens a { adapter } store through Brain.openWithAdapter, which never
installs the int8: 'number' parser that this PR adds only to openPostgres. When gmi()'s
memory.store is given a Postgres adapter, the BIGINT columns load as strings. rowToTrace copies
retrieval_count as-is, so the decay update's retrievalCount + 1 concatenates ("4" becomes "41"),
and the corrupted count is then written back by persistAccess.
The new durable-memory path calls openWithAdapter. Its own new doc comment says pg returns BIGINT as
strings unless the adapter is opened with typeParsers, and that an access then appends to the count.
Only openPostgres sets the parser. MemoryStore.rowToTrace assigns row.retrieval_count without
Number(), and persistAccess writes trace.retrievalCount back. So the PR's claim that "a Postgres
Brain reads its integer columns as numbers" fails on the gmi memory.store adapter path.
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution
## Issue description
When a Brain is opened through `Brain.openWithAdapter` on a Postgres adapter, BIGINT columns come back as strings. `gmi()`'s `memory.store: { adapter }` uses that path, so retrieval counts and timestamps load as text and get string-concatenated.
## Fix Focus Areas
- src/cognition/memory/retrieval/store/MemoryStore.ts[1590-1606]
- src/api/runtime/agentCognitiveMemory.ts[276-290]
- src/cognition/memory/retrieval/store/Brain.ts[1048-1071]
## Recommended Fix
In `rowToTrace` (and any other place that reads `strength`, `retrieval_count`, `last_accessed` or `created_at` from a row), wrap the values in `Number(...)` with a finite fallback, so the result no longer depends on the adapter's type parsers. Alternatively, have `openAgentMemoryBrain` refuse a Postgres adapter that was opened without `typeParsers: { int8: 'number' }`, or document that requirement on `memory.store.adapter`.

4. A forgotten user's words can return via an in-flight write 🐞 Bug≡ Correctness
Description
noteContributors only notes session rows for a write's contributorIds, and tombstoneAfter
re-reads the tombstone of the write's primary write.user alone. If forgetUser(B) commits and
deletes while user A's write that files B's words (for example a rolling summary) is still running,
nothing takes back what that write commits for B. This breaks the promise that every write of a
user's memory, writes in flight included, honours the tombstone.
noteContributors calls noteSession for each contributor but holds no chain for them and keeps no
record to check later. tombstoneAfter returns 'clear' based only on write.user. forgetUser
serializes only the forgotten user's own write chain before it commits the tombstone and deletes. So
a write whose primary user is A, carrying B as a contributor, can finish after B's deletion and stay
as 'kept'.
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution
## Issue description
A write that files another user's words (`contributorIds`) is only checked against the primary user's tombstone after it commits. If a contributor is forgotten while the write runs, their words survive.
## Fix Focus Areas
- src/cognition/learning/LearnerSession.ts[567-595]
- src/cognition/learning/LearnerSession.ts[645-715]
## Recommended Fix
Carry the contributor ids on the write target. After the write commits, extend `tombstoneAfter` to check the tombstone of every contributor as well as the primary user (or take each contributor's write chain for the duration of the write). If any contributor is forgotten, take the write back the same way a forgotten primary user's write is undone.

5. A public ledger type lacks TSDoc✓ Resolved
Description
The newly exported ChainCheck type is declared without a TSDoc comment. Its export through the
learning ledger and package entry points leaves consumers without documentation for the public
chain-verification result.
The changed declaration introduces a publicly exported type without documentation, while the
checklist requires every exported symbol to have TSDoc.

6. A public store options type lacks TSDoc✓ Resolved
Description
The newly exported SqlLearningStoreOptions interface is declared without a TSDoc comment. Its
re-exports through the learning store, learning subpath and package root expose configuration fields
without generated API documentation.
The changed interface is publicly re-exported but has no preceding TSDoc, violating the requirement
that every exported symbol be documented.

Qdrant and Pinecone migrations skip the scope-ID form handshake.
156-156: Major · Data Integrity & Integration · Heavy lift
Qdrant and Pinecone migrations skip the scope-ID form handshake.
_scopeIdFormHandshake returns { refused: [], clear: [] } when either adapter lacks scopeIdForms. The Qdrant source and target adapters and the Pinecone source adapter do not define that method. The migration then processes tables without a form check. This causes the failures at MigrationEngine.scope-id-form.test.ts lines 197, 211, and 223.
The early return is at src/cognition/rag/migration/MigrationEngine.ts:157. The comment anchored at line 156 is a SQLite assertion and does not reach this path.
Implement scopeIdForms on the Qdrant source adapter, the Qdrant target adapter, and the Pinecone source adapter. Model them on SqliteSourceAdapter.scopeIdForms and SqliteTargetAdapter.scopeIdForms. The engine must refuse the migration when a source or target cannot report its form. The tests require this refusal before any table is processed.

Implement scopeIdForms on the Qdrant source and target adapters and the Pinecone
source adapter, following the SQLite adapters’ existing behavior. Update
_scopeIdFormHandshake to refuse migration when either adapter cannot report its
scope-ID form, before processing any tables; keep the SQLite assertion in
rowsOf(to) unchanged.

…to another scope id after the call read it
The compare-and-set of memory_update, memory_delete and memory_merge compared the content and tags alone, so a move that committed between the read and the write (a host's refileScope, a Brain's re-file) left the predicate true: the delete took a trace that had left the turn's scopes, and the update wrote the old snapshot's metadata onto the moved row. The predicate now names the scope and scope id the trace was read under, and such a write answers conflict.
…first, and a graph that cannot delete it fails the delete with the rows kept
MemoryStore.deleteTraces and Memory.deleteTraces ignored a failure of the knowledge graph's deleteMemories and then deleted the trace rows, so a copy holding the trace's full text outlived a forgetUser with no row left to find it by. The copies now go before anything of the traces does, and a failure fails the call with every row kept, so a later call, forgetUser's second pass among them, deletes the traces and their copies.
… brain_meta and refuses a source that names no brain into an encoded target
The handshake returned without a decision whenever either adapter lacked scopeIdForms, and the Qdrant adapters, which copy brain_meta and the graph tables from their SQLite sidecar, lacked it. The Qdrant source and target now read the forms of the brains their sidecar holds, counting each as holding traces whose scope ids the two forms read apart, since those traces are in Qdrant, and the target clears a record from its sidecar. A source that names no brain (a Pinecone index, a Qdrant source with no sidecar) is refused into a target that holds the encoded form and traces, and a target that cannot report its forms takes no source brain with the record.
… as text
The Brain's integer columns are BIGINT on PostgreSQL, and a host adapter opened without typeParsers: { int8: 'number' } read them as strings, so a trace's retrieval count was appended to on each access and written back. The store's open now probes a PostgreSQL adapter and refuses one that reads BIGINT as text, naming the option, before the Brain opens.

…cs name
agent(), gmi() and AgentOSConfig read learning: null as unset at run time, as a config read from JSON or YAML writes it, and their docs say so, while the declared type refused null under strict type checking.
…rd keeps it
The bridge called the observer inside the filing its guard runs, so a filing taken back because its user was forgotten by another process while it ran had already fed the user's words to the observer's buffer, which forgetUser does not reach. The observer now runs after the guard reports the filing kept; with no guard it runs before the encode, as before.
…hes it by id from that filer's turns alone
A thread is shared by every turn of the conversation, and a trace with no turnUserId read as its own to every turn with none: an anonymous turn on a learner's session, which names no second user, could update or delete the learner's thread notes by id, and the learner the anonymous turn's. memory_add now records the filing turn's user scope id on a thread note (metadata.threadFiler), the learner's or the session's form, and reachOf answers other_user to a turn of another filer.
…reason than a missing table fails the migration
clearScopeIdForm on the Qdrant, SQLite and PostgreSQL targets dropped every error, so a delete a locked file or a dropped connection refused left the encoded record over the plain ids the migration had copied, and migrate still returned verified: true. A Qdrant sidecar's read of brain_meta read any failure as a brain the sidecar does not hold. Both now ignore only a missing table or column and rethrow the rest, which migrate records in errors.
… blank persona id is refused (#687)
Five docs pages told a host that a configuration refusal comes back "with `CONFIGURATION_ERROR`". That is the name of the enum member. The error's `code` is the string `'SYS_CONFIGURATION_ERROR'`, the value of `GMIErrorCode.CONFIGURATION_ERROR` in `src/core/utils/errors.ts`, so a host that compares `err.code` with `'CONFIGURATION_ERROR'` never matches the refusal.
Each page now names the string a host compares:
- `docs/getting-started/SQL_STORAGE_QUICKSTART.md`: the conversation manager's refusal of a `conversations` or `conversation_messages` table it did not make, whose message names `conversationManagerConfig.tablePrefix`.
- `docs/safety/PROVENANCE.md`: `AgentOS.initialize()` refusing a provenance policy that leaves prefixed conversation tables unprotected.
- `docs/GMI.md`: `AgentOS.initialize()` refusing a learning option that is not valid.
- `docs/AGENCIES.md`: the full runtime refusing an `agencies` bound out of range.
- `docs/PERSONAS.md`: `AgentOS.create()` refusing an inline `personas` list.
`docs/GMI.md` already wrote the member with its string beside it for a fallback failure, as `CONFIGURATION_ERROR` (`SYS_CONFIGURATION_ERROR`); that form stays allowed.
Six source comments the API reference is built from named the member alone as well: the TSDoc of `ConversationManager.initialize()`, `GMIManager.createAgencySeat()`, `AgentOS`'s learning-option check, `assertInlinePersonaDefinitions()` and `validatePersonaSource()`, and a line comment in `MetapromptExecutor`. Each now names the member with its string. `ConversationManager.initialize()`'s TSDoc also names the option as its message does, `conversationManagerConfig.tablePrefix`.
`docs/PERSONAS.md` also said every refusal of an inline `personas` list names "the index or id". A value that is not an array is refused with the type it received in the error's details, and an entry with no usable `id` by its index alone; the page and `assertInlinePersonaDefinitions()`'s TSDoc now say which refusal names what.
An entry whose `id` is blank (empty or whitespace only, such as `id: ' '`) is refused by its index alone, but the page said its id is named "where that id is a non-empty string", and the refusal's message said the entry needs "a non-empty string `id`", which `' '` is. The message now reads `` personas[<index>] must be an object with a string `id` that is not blank. ``, and the page and the TSDoc say blank where they said non-empty.

Summary
Continues #451, which shipped
memory_traces.scope_idat Brain schema version 3, set by every writer, and the importers' deduplication inside one scope id. This pull request carries the rest of the learning increment.Learning gets a durable record. Operators freeze, suspend, set, revert, export, import and forget through host calls on
AgentOS.getLearningLayer()andagent.learning. Memory a learner owns is filed and read under scope ids that keep tenants, users and sessions apart (the GMI's memory, the memory tools, the standalone retriever and the rolling-summary sink), and every write of a user's memory honoursforgetUser's tombstone, writes in flight included.gmi()gets durable memory (memory.store). Every host gets the stability fix behindpersistAccessState. For every host, flag on or off:

A delete for good (deleteTraces, deleteScope) removes the copies before the rows, and a graph that cannot delete them fails the call with every row kept, so a later call finds the same traces. With no Brain, deleteScopefinds a scope's traces in the vector index, those another store filed before a restart included.MemoryStore.hydrateScopeloads one scope's most recent traces by thescope_idcolumn, so a returning user's older memories are recallable however many traces other scopes hold, andMemoryStore.refileScopemoves one scope id's traces to another: every Brain row, soft-deleted rows included, the vectors of the traces the store holds and their knowledge-graph copies, and with no Brain the documents a durable index holds. A Qdrant source or target takes part through its sidecar'sbrain_meta, a source that names no brain (a Pinecone index) is refused into a target that holds the encoded form, and a read of a form or a clear of a record that fails for another reason than a missing table fails the migration.conversationManagerConfig.tablePrefixkeeps the runtime's conversation tables as<prefix>conversationsand<prefix>conversation_messages.
