fix: clarify ladder progress status

Summary

• preserve confirmed ladder submit and cancellation counts across stop, failure, and symbol-change exits
• name the active ladder action in preparation, execution, replacement, and terminal statuses
• replace ambiguous completion text with explicit submitted-order results
