The README Codex paragraph described the pre-0.14.0 story (approve in a terminal, "Codex has no chat-mint"). It now documents what actually ships and was verified on live Codex v0.154:

  • pythia install writes the AGENTS.md harness, registers the write-free MCP approver in .codex/config.toml, and adds the session-start hook in .codex/hooks.json.
  • Trust with /mcp (and /hooks).
  • Approval is the same shape as Claude: the agent runs pythia apply, calls the pythia_approve tool, the developer answers a select Approve / Reject prompt in Codex, the grant is minted and the agent runs apply --confirm itself.
  • Notes the Codex ≥ v0.120 floor, the console fallback, and the danger-full-access warning.

Compatibility table and the install comment now mention Codex too. Docs-only; all README lints pass.

The Codex paragraph still described the pre-0.14.0 story (approve in a
terminal, no chat-mint). It now documents what ships: install writes the
AGENTS.md harness, registers the write-free MCP approver in
.codex/config.toml, and adds the session-start hook; /mcp and /hooks trust
them; and approval is the same shape as Claude — the agent calls pythia_approve,
the developer answers a select Approve/Reject prompt in Codex, the grant is
minted and the agent applies. Notes the Codex >= v0.120 floor, the console
fallback, and the danger-full-access warning. Compatibility table and the
install comment mention Codex too.

PR #15's squash landed only its first commit; the step-by-step setup sections
for both agents (English and Vietnamese) did not make it onto main. This
brings both READMEs to the intended full state — each with a per-agent
Claude Code / Codex setup walkthrough and a pointer from the security section.
