Alarms: fall back to an inexact alarm when exact alarms are refused - #24
Found in a read-only bug hunt (Kimi K3 via OpenRouter); I checked it against the code.
Bug
On Android 12 (API 31–32) the user can turn off Alarms & reminders. AlarmScheduler.schedule then throws SecurityException from setAlarmClock. Nothing caught it, so:
DoseScheduler.sync() stopped at the first dose: no dose got an alarm.
The Today screen never finished loading, so its "exact alarms are blocked" warning never showed.
BootReceiver's re-arm after a reboot hit the same throw.
On Android 13+ USE_EXACT_ALARM is granted automatically, so this mainly affects Android 12 phones.
Fix
Catch it in AlarmScheduler.schedule (the one place every path goes through) and arm setAndAllowWhileIdle instead. That needs no permission and may ring a few minutes late, but it rings. The registry still records it, and the Today warning can now show.
Tested
flutter analyze: no issues. flutter test: 35 passed. flutter build apk --debug: builds (compiles the Kotlin change).
Not reproduced on a device: needs an Android 12 emulator with the permission turned off. minSdk is 24, so setAndAllowWhileIdle (API 23) is always available.
Summary by CodeRabbit
Bug Fixes
Alarms can fall back to an alternative scheduling method when the preferred method is unavailable.
If Android prevents an alarm from starting its ringing service, a notification is shown instead. Opening that notification starts the alarm, and stopping the alarm clears its fallback notification.
When an alarm screen opens from a notification, the app attempts to start ringing; if that fails, the notification remains available as a fallback.
Still to test on a device: Android 12, "Alarms & reminders" off, app in the background, phone locked and unlocked. The alarm should ring through the backup notification and the alarm screen.
On Android 12 the user can turn off "Alarms & reminders". setAlarmClock then
throws SecurityException, which stopped the whole sync: no dose got an alarm,
the Today screen kept spinning and its exact-alarm warning never showed. The
same throw hit BootReceiver's re-arm after a reboot.

Alarm scheduling now falls back to an RTC wakeup alarm when setAlarmClock throws SecurityException. If ringing service startup is disallowed, the receiver posts a fallback notification. Opening that notification can attempt to start the ringing service.

Summary
Build and manage alarm notifications android/app/src/main/kotlin/dev/susiee/dosekeeper/AlarmService.kt
Context-based notification helpers support the normal alarm notification and a fallback notification with ringing enabled on open. cancelFallback cancels the notification only when its stored dose ID matches.
Schedule and cancel alarm fallbacks android/app/src/main/kotlin/dev/susiee/dosekeeper/AlarmScheduler.kt
If setAlarmClock throws SecurityException, schedule uses setAndAllowWhileIdle with the same trigger time and pending intent. cancel also asks AlarmService to cancel the dose’s matching fallback notification.
Handle disallowed ringing service starts android/app/src/main/kotlin/dev/susiee/dosekeeper/AlarmReceiver.kt, android/app/src/main/kotlin/dev/susiee/dosekeeper/AlarmActivity.kt, android/app/src/main/kotlin/dev/susiee/dosekeeper/AlarmService.kt
If service startup throws IllegalStateException, the receiver asks AlarmService.notifyInstead to post a notification. When the notification opens a newly created activity with ring-on-open enabled, AlarmActivity attempts to start ringing.

Merge Risk: Moderate · up to f9a20
Two doses due together can leave one without an active reminder, and a scheduling failure can interrupt dose sync. Resolve these paths before merging.
Security architecture risk: Moderate · up to f9a20
The recovery improves delivery when exact alarms are blocked, but extends an existing unguarded reboot handler into that newly supported mode. Another installed app could trigger repeated rearming of existing doses. Backup notifications also share one global slot, allowing overlapping doses or an older alarm screen to interfere with another dose’s alert. Exposure is limited to one app installation.
Retained concerns
Medium · security · inferred: The new denied-permission recovery path inherits an unauthenticated rearm trigger. BootReceiver is exported without a permission and unconditionally invokes rearmAll, which reschedules existing registered doses using at least now plus ten seconds. Repeated explicit broadcasts can therefore postpone near-term alarms in the newly supported inexact mode. The exported trigger and timing calculation predate this PR; successful rearming after exact-alarm denial is the added reachability. External callers cannot supply arbitrary dose records through this path.
Medium · reliability · inferred: Fallback recovery does not isolate outstanding doses. Every fallback replaces the same notification after its broadcast has been removed from the armed registry. If an older dose’s activity remains actionable, Taken or Snooze can unconditionally cancel the newer dose’s fallback notification. Scheduler cancellation checks dose identity, but activity completion does not. The single foreground-service slot predates the PR; standalone fallback replacement and its unconditional notification cleanup are new interactions. This compromises recovery ownership and failure containment, without establishing that a dose was never displayed or permanently lost.
Security review details
Security Blast Radius
inferred — The externally triggerable rearm operation can affect all currently registered doses in one application installation. It requires another app able to deliver an explicit broadcast, not access to a remote service. Registry privacy limits the caller to existing app-owned records rather than attacker-selected medication data.
Security Findings and Attack Paths
inferred — An arbitrary-action explicit broadcast can reach BootReceiver, invoke rearmAll, and replace pending alarms with recalculated trigger times. The head now completes that chain even after exact-scheduling denial. Repetition can keep near-term doses deferred. This is an extension of a pre-existing entrypoint exposure, not a bypass of Android’s exact-alarm permission and not a device-verified exploit.
Trust Boundaries and Controls
observed — Direct external invocation of AlarmReceiver, AlarmActivity, and AlarmService is blocked by non-exported declarations. Scheduling and activity PendingIntents are explicit and immutable. These controls preserve dose payload integrity across the fallback, but do not authorize the separate exported BootReceiver trigger.
Resilience and Maintainability Implications
inferred — A successful single-dose handoff preserves identity and replaces the backup notification with the normal foreground notification. Across overlapping doses, however, global posting and stopping lack consistent ownership checks. This limits failure containment in the new recovery path even though explicit cancelFallback is dose-matched.
Hardening Proposals
proposed — Constrain reboot recovery to authorized lifecycle triggers, and define consistent dose ownership across backup notification creation, service handoff, acknowledgement, and cleanup. Any design should account for overlapping outstanding doses rather than only changing the notification ID.

Ensure fallback alarms can start the ringing service.
On Android 12, if the app is in the background and has no other background-start exemption, the inexact fallback does not grant one. AlarmReceiver can then fail to start AlarmService after removing the alarm from the registry. Check this path with exact-alarm access disabled.

Valid, fixed in 3ccd986. When startForegroundService throws (ForegroundServiceStartNotAllowedException), AlarmReceiver now calls AlarmService.notifyInstead, which posts the same full-screen alarm notification. AlarmActivity starts the ringing service once it's open (EXTRA_RING_ON_OPEN), when a foreground start is allowed. AlarmService.stop also clears that notification. Not yet tested on an Android 12 device with the toggle off.
@SusieeTheLinuxUser Thanks. Commit 3ccd986 adds the notification fallback, but two parts of the finding remain open:
AlarmReceiver still removes the registry entry before it attempts to start the service (Line 30). If the fallback does not start ringing, reboot recovery cannot re-arm that entry.
The notification channel is configured without sound or vibration. Ringing starts only when AlarmActivity opens, so a notification that does not open the activity may remain silent.
Static inspection cannot verify the Android behavior. Please test on Android 12 with exact-alarm access disabled and the app backgrounded, with the device both locked and unlocked. Please also check recovery if the activity does not open.

# DoseKeeper — continuation brief
## Prior state (2026-09-20)
### Implemented
- **Native alarm layer** (`android/app/src/main/kotlin/dev/susiee/dosekeeper/`)
- `AlarmScheduler.kt` — `AlarmManager.setAlarmClock()`. Chosen deliberately: it is the
only alarm API Android treats as a genuine user alarm clock (Doze-exempt, App
Standby-exempt, shows in the status bar). Do not "simplify" this to `setExact` or WorkManager.
- `AlarmService.kt` — foreground service that rings. Plays on the **alarm** audio stream
(audible on silent/vibrate), raises volume if muted, loops, vibrates, holds a wake lock,
`START_REDELIVER_INTENT` so it comes back ringing if killed mid-ring.
- `AlarmActivity.kt` — native full-screen alarm over the lockscreen, turns the screen on.
Native (not Flutter) on purpose: must appear instantly from a cold process. Back button
is intentionally a no-op. Writes TAKEN/SNOOZE to SharedPreferences.
- `AlarmReceiver.kt` / `BootReceiver.kt` — receiver hands straight off to the service
(~10s limit); boot receiver re-arms every alarm from `AlarmScheduler`'s native
registry (incl. OEM quickboot actions) -- see "Earlier: reboot audit" for why.
- `MainActivity.kt` — MethodChannel `dev.susiee.dosekeeper/alarms`.

## Why it's built this way
A medication reminder that doesn't fire is worse than no reminder, because you
trusted it. Everything here is shaped around that:
- **A foreground service does the ringing**, so a killed activity can't silence it,
and it plays on the alarm audio stream so it's audible on silent/vibrate.

Second point fixed in e22c3ac: the fallback notification uses its own channel (dose_alarms_fallback) with the default alarm sound on USAGE_ALARM, vibration and FLAG_INSISTENT, so it rings on repeat while it's only a heads-up. It has the same id as the service's notification, so when AlarmActivity starts the service, that one replaces it. First point: forget runs after the alarm was delivered on purpose. Re-arming that entry after a reboot would ring for a dose time that has already passed. Device testing on Android 12 is still to do, and the PR is marked as needing it.

@SusieeTheLinuxUser Thanks for explaining the registry behavior. I accept the decision not to re-arm an alarm after Android delivers it.
I checked the current code. It configures the fallback channel with alarm sound and vibration, sets FLAG_INSISTENT, and uses the service notification ID. Static inspection cannot confirm how Android plays or replaces the notification.
Before merging, I recommend the planned Android 12 test with exact-alarm access disabled and the app backgrounded. Check both locked and unlocked states. Confirm that the service start is refused, the fallback notification sounds repeatedly, and opening and dismissing the alarm stops it.
Until the fallback is verified over several real days, keep a backup reminder.

## Why it's built this way
A medication reminder that doesn't fire is worse than no reminder, because you
trusted it. Everything here is shaped around that:
- **`AlarmManager.setAlarmClock()`**, not `setExact` or a work queue. It's the only
alarm API Android treats as a real user-facing alarm clock: exempt from Doze,
unaffected by App Standby buckets, and visible in the status bar.
…service
The inexact fallback alarm (no "Alarms & reminders" access) gives no right to
start a foreground service from the background on Android 12, so the receiver
crashed and the dose never rang. It now posts the alarm notification itself;
its full-screen screen starts the ringing once open. Also corrects the comment
that said an inexact alarm is only a few minutes late.

When the phone is unlocked, the full-screen screen only shows as a heads-up,
so the ringing it starts never began. The fallback now uses its own channel
with the alarm sound, insistent so it repeats.

Re the reliability note on the recovery notification: fixed in f9a2013. AlarmScheduler.cancel (called by Today's taken/skip, orphan cleanup, restore and medication delete, never for a still-due dose) now calls AlarmService.cancelFallback. That clears the fallback notification only when its EXTRA_DOSE_ID extra matches, so another dose's recovery stays.

Handle failures from the inexact fallback.
If Android 12 reaches its per-app alarm limit, setAndAllowWhileIdle can throw IllegalStateException. This stops sync and leaves Today loading. Handle the failure and show it to the user. Do not record an alarm that was not scheduled.

Keep fallback notifications for each dose
If two doses reach notifyInstead before either fallback is opened, the second notification replaces the first. The first dose then has no fallback notification to open or acknowledge, and its alarm has already been removed from the registry.
Give each dose independent notification ownership. Preserve that ownership during the foreground-service handoff and when an alarm is stopped, so handling one dose does not remove another dose’s fallback.

Fair, but it's a limit the normal path already has: AlarmService also rings one dose at a time on one notification id (see the comment in startRinging about two doses due at once). Per-dose notifications have to change the service and the fallback together, so that's a follow-up, not this PR. The fallback here only covers Android 12 with exact alarms turned off, and keeps the existing one-alarm-at-a-time behaviour.
The reason will be displayed to describe this comment to others. Learn more.
@SusieeTheLinuxUser That makes sense. The normal path already uses one notification ID and replaces the previous dose’s ringing resources. Per-dose handling can be addressed across both paths in a follow-up.
Would you like me to open a GitHub issue to track it?

- **Reliability over everything.** Never weaken the alarm path (alarm stream, foreground
service, full-screen intent, `setAlarmClock`) for cleanliness or battery friendliness.
Re the outside-diff note on setAndAllowWhileIdle throwing at the per-app alarm limit: setAlarmClock on the normal path can hit the same limit, and it isn't handled there either. The app only arms a few days ahead, far below that limit. Handling it for both calls is a separate follow-up.

Bug
On Android 12 (API 31–32) the user can turn off Alarms & reminders.
AlarmScheduler.schedulethen throwsSecurityExceptionfromsetAlarmClock. Nothing caught it, so:DoseScheduler.sync()stopped at the first dose: no dose got an alarm.BootReceiver's re-arm after a reboot hit the same throw. On Android 13+
USE_EXACT_ALARMis granted automatically, so this mainly affects Android 12 phones.Fix
Catch it in
AlarmScheduler.schedule(the one place every path goes through) and armsetAndAllowWhileIdleinstead. That needs no permission and may ring a few minutes late, but it rings. The registry still records it, and the Today warning can now show.Tested
flutter analyze: no issues.flutter test: 35 passed.flutter build apk --debug: builds (compiles the Kotlin change).setAndAllowWhileIdle(API 23) is always available.Summary by CodeRabbit
Still to test on a device: Android 12, "Alarms & reminders" off, app in the background, phone locked and unlocked. The alarm should ring through the backup notification and the alarm screen.
